PhysioNearby helps people find physiotherapists and related practitioners. To do that we have to ask about your health, which is information the law treats with particular care. This policy explains what we collect, why, who sees it, and what you can do about it.
PhysioNearby is a trading name of PHYSIOLABS X LTD, a private limited company registered in England and Wales under company number 15917723, with its registered office at 5th Floor, 167-169 Great Portland Street, London W1W 5PF. For the purposes of UK data protection law, PHYSIOLABS X LTD is the data controller for information you submit through physionearby.uk.

Health information and any clinical documents are "special category" data under Article 9 of the UK GDPR. We treat them accordingly, and we only ask for them because they are what allows a clinician to judge whether we can help you.
Our host records standard server logs, including IP addresses, needed to serve the site securely and to filter spam. Your browser's local storage holds a small counter that limits how many requests can be sent from one device in a day. That counter never leaves your device.

We do not use your information for automated decision-making or profiling. A person reads every request. We never sell your data.
The point of the service is introducing you to someone who can help, so we share your request with the practitioners we think are a good fit. That includes your contact details and the health information relevant to your care. They contact you directly, and from that point they are responsible for their own handling of your data under their own privacy notice and professional obligations.
We share your request with as few practitioners as necessary. If you would like to know exactly who received it, ask us and we will tell you.
We disclose your information otherwise only where the law requires it, or where it is necessary to protect someone from serious harm.
Once a practitioner takes you on as a patient, their clinical record of your care is separate from ours and is kept under the retention rules that apply to healthcare records. Ask them directly about it.
Our host processes data in the United States. Where personal data leaves the UK we rely on the UK International Data Transfer Agreement or the UK Addendum to the EU Standard Contractual Clauses, together with our providers' own safeguards, to give your information a level of protection essentially equivalent to UK law.

Withdrawing consent stops any further use of your information and means we delete your request. It does not undo anything that already happened, such as an introduction already made to a practitioner. If a practitioner has already contacted you, ask them separately to delete what they hold.
When you first arrive we ask which categories you are willing to allow. Nothing outside the essential category is stored on your device until you choose, and you can change your mind at any time through the Cookie settings link in the footer of every page.
All of this is stored locally in your browser. Your choice is recorded with the date you made it, so we can show that consent was given. Clearing your browser storage resets it and we will ask again.
Analytics and advertising tags are delivered by Google Tag Manager and run under Google Consent Mode v2. If you decline a category, the matching tag still loads but is blocked from storing or reading anything on your device: it sends an anonymous, cookieless signal carrying no identifier, which we use only as a page count. Declining advertising also redacts advert-related identifiers before anything is sent.
The site is served over HTTPS, and submissions are transmitted encrypted. Access to submitted requests is limited to the people who need it to run the service. No system is perfectly secure, so please do not send more clinical detail than is useful, and do not use this form for anything urgent.

We may update this policy. The date at the top shows when it last changed. Where a change materially affects how we use information you have already given us, we will contact you.
